Print generic forms for camera grading that do not include specific student names. Students will bubble in their own Student IDs on the form. A Generic Form may be linked to a specific assignment or a class. See additional details below.
When you scan the form, the results will automatically reflect in the assignment you're scanning into for the student. If your student is rostered in more than one class, you will be prompted to select the applicable class. You can bypass the prompt by selecting the specific class in the assignment filters.
For Best Results:

Print forms single-sided on white paper.

Make sure the forms are dark enough that the black outline of the form is not compromised.

Laminate the forms (or use sheet protectors) and reuse them with dry-erase or vis-a-vis markers.
- If you have students rostered to you across multiple classes, consider using pre-filled forms.
Generating Generic Forms for Any Class (ID bubbles added)
Select the tile Print Forms tile from the home page. (You may also select the Forms icon from the toolbar within a specific assignment.) | |
Leave the Assignment field blank to use this form for any assignment that follows the same format or question options (works for multiple choice only)

Select the GENERATE link at the bottom of the form preview to print.

Example Generic Forms
Generic Form for Any Assignment or Class (Generic Form)
- In the Assignment field, (leave blank for generic form). In the Class (optional) field, leave blank so no names are printed on the form and an area is created where students can fill in their student ID numbers (GradeCam ID).
- Leave the default set to Exam from the Form type options
- Type the number of questions you want to appear on the form.
- Select the number of Student ID digits that are needed on the form for students to fill in their student ID number.
- You may change the size of the key on a page by adjusting the %. Remember to generate and download to actually view the adjustment as the preview may not accurately show the change.
- Once your Form Printing selections have been made, click the blue GENERATE button at the bottom of the page.
Specific Assignment Form for Any Class (Generic Form)
- In the Assignment field, identify the specific assignment.
- Leave the Class (optional) field blank so that no names are printed on the form and an area is created where students can fill in their student ID numbers (GradeCam ID).
- Select the number of Student ID digits that are needed on the form for students to fill in their student ID number. If student IDs are 6 digits and you select 3 Student ID digits, have the students bubble the last 3 digits of their IDs.
- You may change the size of the key on a page by adjusting the %. Remember to generate and download to actually view the adjustment as the preview may not accurately show the change.
- Once your Form Printing selections have been made, click the blue GENERATE button at the bottom of the page.

Compatible Assignment Form for Any Class (Generic Form)
- Once a specific assignment has been selected, choose Print a form... to select For assignments with a compatible key
- In the Class field, leave it blank so that no names are printed on the form and an area is created where students can fill in their student ID numbers (GradeCam ID).
- Select the number of Student ID digits that are needed on the form for students to fill in their student ID number. If student IDs are 6 digits and you select 3 Student ID digits, have the students bubble the last 3 digits of their IDs.
