Print paper forms that have pre-filled student names with form identifiers so that students can quickly respond to an assignment or reuse a form across multiple assignments without having to bubble their IDs.

  • For best results, print forms on white paper, single-sided.

  • Make sure the forms are dark enough that the black outline of the form is not compromised.

  • Laminate the forms (or use sheet protectors) and reuse them with dry-erase or vis-a-vis markers.

 

 

Forms with a Pre-Filled Class (ID bubbles removed)

From the home page, select the Print Forms tile. Home Page - Schools and Districts.png

By default, the form will print generically for any assignment.

  • To make these forms specific to an assignment you've already created, start typing the Assignment Name (see image to the right) or move to the next step.

In the Class field, select the class(es) and in the Print forms for... field choose which student(s) to print forms for (options include Any, All, or Select Students).

  • Selecting students will create forms with student names on them with customized form identifiers rather than the GradeCam ID bubble. columns.

When the formatting is complete, select GENERATE. Download the form as a PDF to print or a PNG image to add to an existing document.

Example Scan Forms

Generic Assignment Form for a Pre-Filled Class (multiple choice assignments only)

Select the class(es) or student(s) you want to pre-fill on the generic forms.

  1. Leave the Form type field default drop-down set to Exam.

  2. In the # Questions field, type the number of questions you want to appear on the form.

  3. Type in the characters that should appear as answer choices in the Choices field.

  4. When the formatting is complete, select GENERATE. Download the form as a PDF to print or a PNG image to add to an existing document.

Specific Assignment Form for Pre-Filled Class

Type the name of the specific assignment and select the class(es) or student(s) you want to pre-fill on the forms.

  1. Set the Print a form... field to Only for this assignment.

  2. Set the Print forms for... field to All students (pre-filled ID).

  3. Adjust the Size % of the form and identify the # of copies per student if needed.

  4. When the formatting is complete, select GENERATE. Download the form as a PDF to print or a PNG image to add to an existing document.

Compatible Assignment Form for Pre-Filled Class

Type the name of the specific assignment and select the class(es) or student(s) you want to pre-fill on the forms.

  1. Set the Print a form... field to For assignments with a compatible key.

  2. Set the Print forms for... field to All students (pre-filled ID).

  3. Adjust the Size % of the form and identify the # Copies per Student if needed.

  4. When the formatting is complete, select GENERATE. Download the form as a PDF to print or a PNG image to add to an existing document.
