Generic Forms (ID bubbles added)

Print forms for scanning that do not have pre-filled student id numbers so that you can reuse the forms with multiple assignments, classes and/or students.

For best results, print forms on white paper, single sided. You can print one copy of the form and photocopy it (to save on printer ink costs). If copying from one form, make sure the copies are dark enough that the black outline of the form is not compromised and fits within the margins of the page. Laminate the forms (or use sheet protectors) and reuse them with dry erase or vis-a-vis markers.

Leave the Assignment field blank to use this form for any assignment that follows the format or question options. To make a generic form across classes that is specific to one assignment, type the Assignment Name.

Printing Forms

Preview doesn't always show the updates, so follow the steps below to view the form format per page.

  • Select the Generate link
  • Download the form as a PDF to print or PNG image to add to an existing document (see samples here)
  • Example Scan Form - Generic Assignment for a Any Class

    1. In the Class field select Print generic form for any class so no names are printed on the form and an area is created where students can fill in their student id numbers.
    2. Select Exam from the Form type options.
    3. Type the number of questions you want to appear on the form.
    4. Select the number of Student ID digits that are needed on the form for students to fill in their student id number.
    5. Type in the characters you want to appear in the answer choice bubbles.

    1. Once a specific assignment has been selected, choose Print a form... to select For assignments with a compatible key
    2. In the Class field select Print generic form for any class so no names are printed on the form and an area is created where students can fill in their student id numbers.
    3. You may change the size of the key on a page by adjusting the %. Remember to generate and download to actually view the adjustment as the preview may not accurately show the change.
