Never print another form! Well at least this semester :)
1) Print out an exam form, preferably with the roster all ready filled in.
2) Laminate
3) Distribute to your students. Now they can take tests with a sharpie and re-use their forms.
Also, no id is necessary. You can have them bubble their id in on a blank form. Then you really have a low consumption solution.

We do this in our school district and has worked really well. The only issue we have had with this is the lights above the camera creating a glare and not allowing the sheet to be read. If the lights are off and just use light from the window it scans just fine.
A sharpie is a permanent marker. Use a Vis-a-vis (an erasable marker) just tell the students to be careful and to fill in the bubbles lightly; when there is too much ink, the program does not read it.
If you use a low glare laminate, your forms will read better under bright lights. If you have problems reading the forms under bright lights, the camera will also.
